ATTENTION ALL RESIDENT STUDENTS
The College has engaged the services of an insurance company to cover losses to your personal property. This would include losses due to theft, breakage, water damage such as sprinkler systems, etc.
The cost for all residents students is $45 per semester ($90 total) and will cover up to $4,000 of property per incident with a $50 deductible. However, you are not required to purchase this insurance. This form allows you to waive/opt out of this personal property insurance plan.
If you do not want to participate in this plan, please read the following statement and submit it electronically and your record will be kept on file:
I choose to not participate in the CSI Personal Property Insurance Plan and decide to waive/opt-out of this plan. I understand that by opting out of this insurance plan, I will receive no benefits from this plan, will not have any insurance coverage for my personal property by CSI Insurance, and that Bloomfield College is not responsible for loss, damage, theft, etc. of my personal belongings under any circumstances.

Policy Opt-Out Dates: Students will have up to 30 days to opt out of the program. Students must opt out by September 19, 2008 for the first premium and by February 21, 2009 for the second premium.
